Medicare Wound-Care Coding: Separate Code, Coverage and Payment

An October 2026 guide to wound-care purchasing that separates HCPCS identity, coverage conditions, payment settings and future competitive-bidding dates.

Medicare Wound-Care Coding: Separate Code, Coverage and Payment

Summary

A wound-care purchasing worksheet can contain a valid HCPCS code and a genuine Medicare amount yet still support the wrong commercial conclusion. The code may describe an ocular device, the amount may apply only to the first rental months, or the payment may belong to home health rather than the DME benefit. For late-October 2026 planning, the useful task is to connect product identity, coverage, care setting, payment method, and effective date before comparing figures. The sources below include current October changes and earlier rules still relevant to that decision; they are not all newly issued this month.

Four checks organize the analysis. First, distinguish durable wound NPWT code E2402 and its supplies from the newly added external ocular negative-pressure codes E2403 and A6614. Second, separate the October 28 Master List additions from actual F2F/WOPD and prior-authorization selections. Third, use the current Round 2028 category, lead-item, and timetable publications rather than an older competitive-bidding recommendation. Fourth, identify the $282.10 CY 2026 home-health disposable NPWT device rate by its benefit setting, rather than relabeling it as a universal DMEPOS or physician-office allowance 2 1 7 8.

These distinctions matter to suppliers and hospital value-analysis teams because a reimbursement amount is not a device acquisition price, a coverage decision, or a guarantee of payment on an individual claim. A manufacturer or distributor should provide accurate product and coding evidence, while the billing entity verifies the beneficiary, setting, applicable policy, modifiers, and documentation. The article supplies a source-based comparison and an internal verification method, not an individual coverage determination.

Scope and Pricing Guardrail: Public Medicare fee-schedule cells (DMEPOS_OCT.csv, DMEPOS_JUL.csv), CBIC Round 2028 bid limits, and the Transmittal 13488 Table 8 Home Health dNPWT rate are statutory/regulatory Medicare reimbursement allowances for covered beneficiaries under specific benefit categories (Part B DMEPOS, Part B Surgical Dressings, or Home Health PPS). None of these figures is a hospital acquisition cost, a distributor tender price, or a VEMERIX product selling price 4 5 7 8.

1. The October 2026 DMEPOS Quarterly Update (MM14619 and DME26-D): Separating the Wound-Therapy Pump (E2402 / A6550 / A7000) from the New Ocular Pump (E2403 / A6614)

1.1 What MM14619 (CR 14619, Transmittal R13956CP) Changed on October 1, 2026

On September 29, 2026, CMS released Medicare Learning Network (MLN) Matters Article MM14619 (DMEPOS Fee Schedule: October 2026 Quarterly Update, implementing Change Request 14619 and Transmittal R13956CP, with an effective date of October 1, 2026 and MAC implementation date of October 5, 2026) 2. Simultaneously, CMS published the fourth-quarter DMEPOS fee schedule package (dme26-d.zip, containing DMEPOS_OCT.csv with 3,538 fee rows, up from 3,531 rows in the third-quarter DMEPOS_JUL.csv file) 4 5.

On page 2 of MM14619, CMS announces three structural code actions for the fourth quarter of CY 2026 2:

  1. Seven HCPCS Level II Codes Added Effective October 1, 2026: A4228, A6614, E0788, E2403, L1330, L1972, and L8697 2.
  2. One HCPCS Level II Code Deleted Effective September 30, 2026: L8696 (with zero additions or deletions to the Parenteral and Enteral Nutrition [PEN] fee schedule file) 2.
  3. Six HCPCS Level II Codes Receiving New DMEPOS Fee Schedule Amounts Effective October 1, 2026 (following public consultation during the First Biannual 2026 Non-Drug and Non-Biological Items and Services HCPCS Code Application Review Cycle under 42 CFR §§ 414.114 and 414.240): A4228, A6614, E0150, E0788, E2403, and L1972 2.

Crucially, MM14619 prints only the bare five-character alphanumeric codes in its bulleted lists and instructs billers to "Refer to the narrative summary for the Medicare benefit category and payment determinations for these items" (MM14619, p. 2) 2. Similarly, DMEPOS_OCT.csv contains no description column: Column 1 is HCPCS, Column 2 is Mod, Column 3 is Mod2, Column 4 is JURIS, Column 5 is CATG, Column 6 is Ceiling, Column 7 is Floor, and Columns 8 through 113 contain the 53 non-rural (NR) and 53 rural (R) state and territory fee cells (49 contiguous states/DC plus AK, HI, PR, and VI, as defined in DMEREAD 2026 Attachment A-1) 4 5.

1.2 Why E2403 ($1,352.58 Monthly Rental) and A6614 ($167.99 Monthly Supplies) Must Never Enter a Wound-Pump Bid

When a reimbursement analyst or procurement specialist opens DMEPOS_OCT.csv and scrolls to the E2400 series, E2403 (Mod = RR, JURIS = D, CATG = CR) sits on the row immediately below the negative pressure wound therapy electrical pump E2402 (Mod = RR, JURIS = D, CATG = CR) 4:

  • Both E2402 and E2403 carry Modifier RR (Rented) 4 5;
  • Both carry Jurisdiction D (DME MAC jurisdiction) 4 5; and
  • Both carry Payment Category CR (Capped Rental, governed by the 13-month capped rental rules of Social Security Act § 1834(a)(7) and 42 CFR § 414.229) 4 5.

Without the HCPCS narrative descriptor, a spreadsheet user who sees E2402 RR (contiguous non-rural range $748.28 to $881.94, median $829.21; contiguous rural $1,504.41) followed immediately by E2403 RR (Ceiling $1,352.58, Floor $1,149.69, uniform contiguous non-rural $1,352.58, contiguous rural $0.00) can easily conclude that CMS created E2403 on October 1, 2026 as a higher-tier wound-therapy pump code or a replacement for E2402 2 3 4. Likewise, seeing A6614 (Mod = NU, JURIS = D, CATG = IN, Ceiling $167.99, Floor $142.79, uniform non-rural $167.99) near the wound-care dressing set code A6550 ($31.08–$31.92 non-rural; $33.70 rural) invites the false assumption that A6614 is a bundled monthly NPWT wound-dressing kit 3 4.

Opening the Joint DME MAC and PDAC October 2026 HCPCS Code Update — Correct Coding bulletin (published October 1, 2026, the preserved primary-source extract) immediately refutes that assumption 3. Under the heading Added Codes — Miscellaneous, the DME MACs and PDAC publish the exact official narratives effective for dates of service on or after October 1, 2026 3:

  • E2403: "EXTERNAL OCULAR NEGATIVE PRESSURE PUMP, ELECTRIC" 3
  • A6614: "SUPPLIES AND ACCESSORIES FOR USE WITH AN EXTERNAL OCULAR NEGATIVE PRESSURE PUMP, ANY TYPE, PER MONTH" 3

E2403 describes an electric external ocular negative-pressure pump; A6614 describes monthly supplies and accessories for such a pump. Those descriptors identify an ocular category, not wound NPWT. The coding bulletin also cautions that inclusion of a code does not establish coverage. This article does not infer a clinical indication or mechanism from the descriptor beyond what is needed to distinguish the coding categories 3.

Table 1 compares selected wound-care/NPWT codes (E2402, A6550, A7000, A6022, A6197), the adjacent urine/fecal suction pump code (E2001), and two of the six codes receiving new October 2026 fee amounts (E2403 and A6614) across the 49 contiguous U.S. non-rural and rural state columns. It does not reproduce all newly priced codes 1 2 3 4 5.

Rental-stage qualification: the displayed capped-rental base amount applies to months 1–3; months 4–13 use 75% of that amount under the applicable capped-rental rules. For E2403, $1,352.58 therefore becomes $1,014.44 after rounding for months 4–13. Apply the corresponding rental-stage rule to E2402 as applicable. A worksheet should record rental month and eligibility, not repeat the initial amount as though it applied unchanged throughout the rental period. The table reports fee-schedule allowances, not net Medicare disbursements; deductible and coinsurance rules must be handled separately 4 5 13.

1.3 Why Ceiling and Floor Are $0.00 on E2402, A6550, and A7000 While Populated on E2403, A6614, A6022, and A6197

A second recurring misreading in DMEPOS_JUL.csv and DMEPOS_OCT.csv involves Columns 6 (Ceiling) and 7 (Floor). As shown in Table 1, E2402 RR, A6550, and A7000 NU display $0.00 in both the Ceiling and Floor columns, whereas E2403 RR displays a $1,352.58 Ceiling and $1,149.69 Floor (Floor = 85.0% of Ceiling), A6614 NU displays $167.99 / $142.79, A6022 displays $29.97 / $25.47, and A6197 displays $23.44 / $19.92 4 5.

The CMS file documentation explains how to interpret the fee fields. A zero in a ceiling, floor, rural, or non-rural field is not by itself a coverage decision or a universal zero allowance. Applicable methodology depends on the item category and geography. The former-CBA ZIP file identifies geography; it is not a payment-amount table. Map the beneficiary's location, category and modifiers to the relevant amount and governing methodology. Do not extend one adjustment rule, including 414.210(g)(10), indiscriminately to all non-rural, rural and former-CBA situations 2 5 13.

For example, the displayed E2402 and supply fields reflect prior competitive-bidding adjustments, while other rows use different fee-setting rules. National ceiling/floor fields are not universally applicable to every DMEPOS category. The correct question is which published field and rule applies to this claim setting, not which nonzero cell gives the highest amount. Keep the geography lookup and the amount lookup as separate, auditable steps.

2. The October 28, 2026 DMEPOS Master List, F2F/WOPD List, and Prior Authorization Update (91 FR 47972, Doc. 2026-15446): Why A6022 and A6197 Wound Dressings Are Added to Table 1—and What That Does and Does Not Require

2.1 The Three-List Statutory and Regulatory Architecture (42 CFR §§ 410.38 and 414.234)

On July 30, 2026, CMS published Federal Register Document 2026-15446 (Medicare Program; Updates to the Master List of Items Potentially Subject to Face-to-Face Encounter and Written Order Prior to Delivery and/or Prior Authorization Requirements; Updates to the Required Face-to-Face Encounter and Written Order Prior to Delivery List; and Updates to the Required Prior Authorization List, 91 FR 47972–47978, Docket CMS-6109-N, RIN 0938-ZC04), with an implementation date of October 28, 2026, except that prior authorization for upper-limb orthoses follows the geographic phases described below (91 FR 47972) 1 13.

Because industry alerts frequently conflate "being placed on the CMS Master List" with "requiring Medicare Prior Authorization," hospital wound-care clinics and surgical dressing distributors must first understand how the November 8, 2019 Final Rule (84 FR 60648, effective January 1, 2020) and 42 CFR §§ 410.38 and 414.234 structure three distinct lists under Social Security Act §§ 1834(a)(1)(E)(iv), 1834(a)(11)(B), and 1834(a)(15) (91 FR 47972–47973) 1 13:

  1. List 1 — The Master List (42 CFR § 414.234(b)):

The Master List is a potential-vulnerability library, not an active claims-denial edit (91 FR 47972) 1. As CMS states verbatim on page 47972:

"The Master List serves as a library of items, that have been identified as potential vulnerabilities to the Trust Fund based on criteria outlined in 42 CFR 414.234(b), from which items may be selected to be placed on either the Required Face-to-Face Encounter and Written Orders Prior to Delivery List (the 'F2F/WOPD List') and/or Required Prior Authorization List under the authority provided under sections 1834(a)(1)(E)(iv), 1834(a)(11)(B), and 1834(a)(15) of the Social Security Act (the Act). Only those items that are selected and announced via Federal Register notice are subject to such regulatory conditions of payment." 1.

Prior to the October 28, 2026 update, the Master List (last updated on January 13, 2026 at 91 FR 1250–1252) contained 530 items (91 FR 47973). Adding 20 codes in Table 1 (91 FR 47974–47975) with zero removals brings the Master List to 550 items as of October 28, 2026 1.

  1. List 2 — The Required Face-to-Face Encounter and Written Order Prior to Delivery (F2F/WOPD) List (42 CFR § 410.38):

Items selected from the Master List and placed on the F2F/WOPD List require, as a condition of Medicare payment, both a documented face-to-face clinical encounter (in-person or qualifying telehealth under 42 CFR §§ 410.78 and 414.65) within the six months preceding the order and a Written Order Prior to Delivery (WOPD) communicated to the supplier before the item is delivered (42 CFR § 410.38(d), 91 FR 47975) 1 13. Prior to October 28, 2026, the F2F/WOPD List (91 FR 1252–1253) contained 83 items (including 46 statutorily required Power Mobility Devices [PMDs]). Adding 22 codes in Table 2 (91 FR 47975–47976) brings the F2F/WOPD List to 105 items on October 28, 2026 1.

  1. List 3 — The Required Prior Authorization List (42 CFR § 414.234(c)):

Items selected from the Master List and placed on the Required Prior Authorization List require the supplier to submit a prior authorization request with medical-necessity documentation and obtain a provisional affirmative prior authorization decision from the DME MAC before furnishing the item and billing Medicare (91 FR 47976) 1 13. Prior to October 28, 2026, the Required Prior Authorization List (91 FR 1253–1254) contained 74 items. Adding 8 codes in Table 3 (91 FR 47976–47977) brings the Required Prior Authorization List to 82 items (with an estimated $15.8 million in net Medicare savings after implementation costs, 91 FR 47978) 1.

2.2 Why a low unit price does not prevent Master List inclusion

How did two sterile wound dressing codes with single-unit fee schedule ceilings of $29.97 (A6022) and $23.44 (A6197) enter the CMS Master List in Table 1 (91 FR 47974) when the Master List's headline cost threshold is $614 for purchases and $62 for monthly rentals?

The answer lies in the two pathways relevant to these additions codified at 42 CFR § 414.234(b)(1)(i)–(ii) and explained on pages 47973–47974 of 91 FR 47972 1 13:

  • Pathway 1 — Inflation-Adjusted Cost/Volume Threshold + OIG/GAO/CERT Report (42 CFR § 414.234(b)(1)(i)):

An item on the DMEPOS Fee Schedule qualifies for the Master List if it has an average purchase fee of $614 or greater in CY 2026 (calculated by applying the +2.0% CY 2026 update factor [2.7% 12-month CPI-U ending June 30, 2025 minus 0.7% BLS private nonfarm business total factor productivity (TFP)] to the CY 2025 $602 threshold = $614.04, rounded to $614), OR an average monthly rental fee of $62 or greater in CY 2026 ($61 × 1.02 = $62.22, rounded to $62), OR accounts for at least 1.5% of all Medicare DMEPOS expenditures over a recent 12-month period, AND is identified in a national GAO or HHS OIG report published in 2015 or later (updated to reports published after 2020 in maintenance cycles) or listed in a CMS Comprehensive Error Rate Testing (CERT) Medicare Fee-for-Service Supplemental Improper Payment Data report as having a high improper payment rate (91 FR 47973–47974) 1 10 11. Twelve of the 20 codes in Table 1 (A4594, E0658, E0659, E0683, E0734, E0738, E0739, E2001, E2298, L1933, L1952, and L5827) entered via this pathway (91 FR 47974) 1.

  • Pathway 2 — Aberrant Billing Pattern Rule (42 CFR § 414.234(b)(1)(ii), No Minimum Unit-Price Threshold):

Even when an item's unit purchase price is well below $614, 42 CFR § 414.234(b)(1)(ii) directs CMS to add to the Master List any DMEPOS Fee Schedule item that had:

  1. At least 1,000 claims and at least $1,000,000 in Medicare payments during a recent 12-month period (July 2024 through June 2025 for the CY 2026 update);
  2. Payments during that 12-month window that exceeded payments in the preceding 12 months by the greater of double the percentage change of all qualifying DMEPOS claim payments OR more than a 30 percent increase in payments; and
  3. No explanatory contributing factors (such as a new technology launch or newly expanded coverage policy) (91 FR 47973–47974) 1 13.

The rounded 2026 purchase threshold remains $614. The arithmetic $602 × 1.02 equals $614.04; a different unrounded figure in the notice should not be repeated as a correct calculation. A separate statutory category also exists under 414.234(b)(1); these two pathways are not an exhaustive statement of every route to the Master List. In addition, the 49-area median displayed in this article is descriptive and is not proof that CMS's statutory average fee threshold was met 1 13.

On page 47974 of 91 FR 47972, CMS explicitly names the eight HCPCS codes added to the Master List in Table 1 under this aberrant billing pattern (> 30% annual payment surge) criterion 1:

  1. A4239 (Supply allowance for non-adjunctive, non-implanted continuous glucose monitor [CGM] including all supplies and accessories, 1 month supply = 1 unit; $273.28 ceiling);
  2. A6022 (Collagen dressing, sterile, size more than 16 sq in but less than or equal to 48 sq in, each; $29.97 ceiling);
  3. A6197 (Alginate or other fiber gelling dressing, wound cover, sterile, pad size more than 16 sq in but less than or equal to 48 sq in, each; $23.44 ceiling);
  4. K0831 (Power wheelchair, group 2 standard, seat elevator, captain's chair, up to 300 lbs);
  5. L0456 (Thoracic-lumbar-sacral orthosis [TLSO], flexible, prefabricated, customized);
  6. L0486 (TLSO, triplanar control, two-piece rigid plastic shell, custom fabricated);
  7. L1833 (Knee orthosis, adjustable knee joints, positional orthosis, rigid support, prefabricated, off-the-shelf); and

Across the three additions tables, the union is 35 codes, not 31: 20 Master List additions plus 22 F2F/WOPD additions minus seven overlapping additions, with the eight prior-authorization additions already within that union. The comparison's purpose is to identify the operative list and effective date. Retention is conditional under the regulation, and prior authorization can involve geographic phases, exceptions, or exemptions. Check the operative rule for the actual supplier and item rather than converting a table label into an unconditional national claim 1 13.

3. DMEPOS Competitive Bidding Program (CBP) Round 2028: Why the Official Category List (December 8, 2025) and CBIC Lead-Item Table (June 4, 2026) Do Not Include an NPWT Category

3.1 A historical recommendation does not define Round 2028

In hospital tender specifications and supplier reimbursement briefings for durable negative pressure wound therapy pumps (E2402), an analyst may encounter the HHS Office of Inspector General (OIG) report Comparison of Prices for Negative Pressure Wound Therapy Pumps (OEI-02-07-00660), which recommended that CMS include E2402 pumps in the DMEPOS Competitive Bidding Program (CBP). While CMS did include NPWT in earlier competitive-bidding rounds (which is why E2402, A6550, and A7000 carry competitively adjusted non-rural and rural fee columns and $0.00 national ceiling/floor fields in DMEPOS_JUL.csv and DMEPOS_OCT.csv, s4, s5), all Round 2021 CBP contracts expired on December 31, 2023, placing the DMEPOS CBP into an extended gap period starting January 1, 2024 (MM14619, p. 3) 2 4 5. The OIG report was published in March 2009 using 2007 data; its identifier is not its publication year. Its historical recommendation is not evidence of the current Round 2028 category list 16.

When a distributor or hospital procurement officer asks in October 2026 whether E2402 NPWT pumps are included in the next round of competitive bidding (Round 2028), that question cannot be answered by citing a March 2009 OIG recommendation. It must be answered by reading the two official bidder-education documents whose express regulatory duty is to enumerate the product categories and lead items for Round 2028:

  1. The CMS Fact Sheet, Durable Medical Equipment, Prosthetics, Orthotics, and Supplies Competitive Bidding Program — Updates and Important Information (updated December 8, 2025, reflecting the CY 2026 Home Health Prospective Payment System Final Rule CMS-1828-F displayed in the Federal Register on November 28, 2025, the preserved primary-source extract) 6; and

3.2 The Seven Named Round 2028 Product Categories, Their Seven Lead Items (CBIC), and the Nationwide Remote Item Delivery (NRID) Architecture

The October 7, 2026 official Round 2028 timeline supersedes the earlier approximate registration/bidding dates for planning purposes: registration opens December 1, 2026 and closes February 1, 2027; bidding opens January 5, 2027 and closes March 10, 2027. Preserve the distinction between registering and submitting a bid. The program's transition provisions should not be described as a blanket six-month grace period: the applicable order and documentation conditions still matter, and the provision does not apply uniformly to every item category 14 6.

Under the heading Next Round DMEPOS CBP Product Categories in the December 8, 2025 CMS Fact Sheet (s6) and the June 4, 2026 CBIC Lead Item Bidding and Pricing table (s7), CMS names exactly seven product categories and seven Lead Item HCPCS codes for Round 2028—and designates all seven categories for the Nationwide Remote Item Delivery (NRID) Program 6 7. Table 3 documents the seven Round 2028 product categories, their Lead Item HCPCS codes, their CBIC Bid Limits under 42 CFR § 414.412(b)(11)–(12), their October 2026 DMEPOS_OCT.csv fee cells, and their pricing methodology under 42 CFR §§ 414.414 and 414.416 2 4 6 7.

Bid limits are ceilings for bids, not awarded Single Payment Amounts or beneficiary-specific reimbursements. A lead-item example in a pricing bulletin is illustrative unless identified as an actual award. The category and lead-item enumeration establishes what those documents name; it does not establish that NPWT can never be included in a later program change 7.

3.3 How Round 2028 Lead-Item Pricing (75th Percentile SPA) Works—and What a Tender Writer May and May Not Claim About NPWT (E2402)

As detailed in the June 4, 2026 CBIC Lead Item Bidding and Pricing bulletin (s7) and the current regulatory provisions, Round 2028 uses the lead-item composite pricing methodology codified at 42 CFR §§ 414.414 and 414.416 7 13 13:

  1. Single Lead-Item Bid Entry: A bidding supplier submits a single dollar bid—only for the Lead Item HCPCS code in that product category (A4239, A4296, A4385, L0651, L1852, L3916, or A4352)—at or below the published Bid Limit (42 CFR § 414.412(b)(11)–(12)) 7. Because a contract supplier must furnish every code in the product category, the lead-item bid serves as a composite bid representing the supplier's cost across the entire category 7.
  2. 75th Percentile Single Payment Amount (SPA) Rule (42 CFR § 414.416(b)(1)): Under current regulations (replacing the older median-bid rule used in early CBP rounds), the Single Payment Amount (SPA) for the Lead Item is set at the 75th percentile of bid amounts submitted by suppliers in the winning range for that Competitive Bidding Area (CBA) and product category (CBIC gives the illustrative example of a $191.30 75th percentile winning bid on A4239 against the $273.28 bid limit, representing a 30% reduction) 7.
  3. 2015 Unadjusted Fee Ratio Rule for Non-Lead Items (42 CFR § 414.416(b)(3)): Once the Lead Item SPA is determined, CMS calculates the SPA for every non-lead HCPCS code in that product category by multiplying the Lead Item SPA by a fixed relative ratio based on the 2015 DMEPOS fee schedule amounts (chosen because competitive-bidding adjustments to the national fee schedule began in 2016) 7. The cited 2022 Chapter 36 revision is historical background; it should not be used by itself to establish the current Round 2028 percentile rule 9.

The inspected official categories and seven lead items do not name an NPWT category or an E2402/A6550/A7000 lead item. That is the supported statement. An older OIG recommendation cannot add a category to the current enumeration. This conclusion does not establish a perpetual exclusion from future rounds or independently reproduce every non-lead item in a calculator workbook 6 7.

For current durable NPWT claims, identify the relevant fee methodology and geography under the gap-period rules, then distinguish the allowed amount from Medicare's payment after applicable cost sharing. The former-CBA file is a ZIP classification aid, not the location of a separate dollar amount. A tender should state the rate's category, modifier, geography, rental stage and date, rather than attach a general “Medicare reimbursement” label to one cell 2 5 13.

4. The CY 2026 Home Health Disposable NPWT (dNPWT) Separate Device Rate (Transmittal 13488 / CR 14304, Table 8 = $282.10): Separating Home Health PPS (TOB 032x), Part B DMEPOS (E2402), and the Physician Fee Schedule (97607 / 97608)

4.1 What CMS Transmittal 13488 (CR 14304) Table 8 Actually Establishes for CY 2026

When hospital wound-care directors, outpatient clinics, and home-health agencies compare a reusable durable NPWT pump system (E2402 + A6550 + A7000) against a mechanically or electrically powered single-use / disposable NPWT (dNPWT) device (HCPCS A9272: Wound suction, disposable, includes dressing, all accessories and components, any type, each), commercial reimbursement cards and secondary buyer guides frequently print $282.10 and call it a "2026 Medicare Physician Fee Schedule (MPFS) allowable."

Opening CMS Manual System Pub 100-04 (Medicare Claims Processing), Transmittal 13488 / Change Request 14304 (Home Health Prospective Payment System [HH PPS] Rate Update for CY 2026, issued November 20, 2025, re-communicated December 10, 2025, effective January 1, 2026, the preserved primary-source extract and r13488cp.txt) reveals the exact statutory origin, arithmetic, and benefit label of $282.10 8:

  1. Statutory Authority (CAA 2023, Division FF, Section 4136):

Under Section 4136 of Division FF of the Consolidated Appropriations Act, 2023 (Pub. L. 117-328), Congress reformed how Medicare pays for an applicable disposable negative pressure wound therapy device furnished on or after January 1, 2024 to a Medicare beneficiary under a Home Health plan of care (r13488cp.txt, Section Disposable Negative Pressure Wound Therapy) 8. Prior to 2024, Home Health payments for dNPWT were tied to the Hospital Outpatient Prospective Payment System (OPPS) and included nursing visit time. Starting in CY 2024:

  • The separate payment is for the disposable device only (reported with HCPCS code A9272 on Home Health Type of Bill [TOB] 032x), while the home-health nursing or therapy visit time required to apply or change the device is bundled into the standard 30-day Home Health PPS period payment 8;
  • For CY 2024, the statutory starting point was set equal to the national Physician Fee Schedule (PFS) practice-expense supply price for an applicable disposable device as of January 1, 2022, updated by the applicable CPI-U increase reduced by productivity, as specified for the initial calculation 8; and
  • For CY 2025 and each subsequent calendar year, CMS updates the prior year's Home Health dNPWT payment amount by the percentage increase in the Consumer Price Index for All Urban Consumers (CPI-U) for the 12-month period ending in June of the preceding year, reduced by the multifactor productivity (MFP / TFP) adjustment (r13488cp.txt) 8.
  • Exact Arithmetic of Table 8 in Transmittal 13488 (Transmittal 13488, PDF page 11):

In Transmittal 13488, CMS states that for CY 2026, the 12-month CPI-U increase ending June 2025 is 2.7 percent and the productivity adjustment is 0.7 percentage points, yielding a net update of 2.0 percent (an update factor of 1.02); this is the same 2.7% − 0.7% = 2.0% (× 1.02) numerical update factor described for the 2026 Master List thresholds, although the two applications have different statutory contexts 1 8. Applying that 1.02 factor to the CY 2025 dNPWT payment rate of $276.57 produces Table 8 of Transmittal 13488 8:

Table 8 Field (CMS Transmittal 13488 / CR 14304, Effective January 1, 2026)Official Published ValueStatutory & Formula Notes (CAA 2023 Division FF § 4136)
CY 2025 dNPWT Payment Rate$276.57Prior-year national separate device payment rate on Home Health claims (TOB 032x, HCPCS A9272) 8.
CY 2026 Update Factor1.02 (+2.0%)12-month CPI-U ending June 2025 (2.7%) minus multifactor productivity adjustment (0.7%) = +2.0% 8.
CY 2026 dNPWT Payment Rate$282.10$276.57 × 1.02 = $282.1014, rounded to $282.10 8.

Table 4: Transcription of Table 8 (CY 2026 Disposable Negative Pressure Wound Therapy [dNPWT] Device Rate) from CMS Transmittal 13488 / Change Request 14304 (r13488cp.pdf, effective January 1, 2026) 8.

4.2 Why Mislabeling $282.10 as a "2026 MPFS Allowable" or "DMEPOS Fee" Distorts Procurement Comparisons

The $282.10 amount is the CY 2026 home-health separate disposable NPWT device rate, not a universal physician-office or DMEPOS allowance. The initial statutory calculation referenced a 2022 physician-fee-schedule supply price, but that historical input does not change the current benefit setting. The applicable home-health instructions govern reporting and payment, including the treatment of visit time and applicable beneficiary liability 8.

A9272 describes a disposable wound-suction device and does not qualify as durable equipment under the NPWT DME policy article. E2402, by contrast, describes a qualifying electrical NPWT pump and has product-specific coding requirements. Physician-office and hospital-outpatient services have their own payment systems and service-code instructions; this report does not provide a setting-specific 2026 payment quotation for those services. Obtain the relevant MPFS or OPPS source before inserting an amount in those columns 15.

Table 5 compares selected benefit and coding situations relevant to this purchasing decision. It is not an exhaustive map of all wound-care settings; the ocular entry is an adjacency counterexample.

A skilled nursing facility is excluded from the DME “home” definition even when the stay is not paid under Part A; “LTC non-Part A” is therefore not a sufficient eligibility description. The applicable site and benefit rules must be checked. For E2402, verify the actual product's PDAC coding verification and Product Classification List status rather than assuming every reusable vacuum device qualifies. A6022 likewise has product-specific PDAC/PCL requirements in the surgical-dressing policy 13 15 17.

The NPWT LCD's usual supply limits are 15 A6550 kits per wound per month and 10 A7000 canister sets per month. An exception to the canister limit can be supported for documented drainage greater than 90 ml per day. These limits do not establish coverage without the other criteria, nor authorize a supplier to infer a clinical need from the maximum allowance. Record the actual order and supporting documentation 18.

5. How CMS Uses CERT Improper Payment Data (2024 and November 2025 Supplemental Reports) to Populate the Master List and Prior Authorization Program (91 FR 47972)

5.1 Connecting 91 FR 47972 (Footnotes 5–7) to the 2024 and November 2025 CERT Supplemental Reports

In Section II.C of 91 FR 47972 (page 47977, Footnotes 5, 6, and 7), CMS explicitly cites the 2023, 2024, and 2025 Medicare Fee-for-Service (FFS) Supplemental Improper Payment Data (CERT) reports to explain why CMS selected 8 orthosis, wheelchair, and air-fluidized bed codes (E0194, K0005, L1833, L0456, L0457, L0486, L3761, and L3916) for Prior Authorization (Table 3), starting October 28, 2026 with phased geographic implementation for L3761 and L3916, while leaving NPWT (E2402) off Table 2 and Table 3 1 10 11:

  • As CMS states on page 47977 of 91 FR 47972:
"Orthoses have been identified by the CERT program as one of the top DMEPOS service types with improper payments over the past several years. From 2023 to 2025, improper payment rates remained consistently elevated, ranging from approximately 40 percent to 48 percent for upper limb orthoses and 35 percent to 47 percent for lower limb orthoses. The improper payment rate for manual wheelchairs ranged from 22.1 percent to 42 percent over that same time period." 1.
  • In addition, CMS notes on page 47977 (Footnotes 3 and 4) that the CMS Fraud Defense Operations Center (FDOC) reported over $1.5 billion in payments suspended for DMEPOS items associated with suspected fraudulent billing, and cites U.S. Department of Justice (DOJ) enforcement actions involving telemarketing and medically unnecessary orthoses 1.

5.2 What the 2024 and November 2025 CERT Supplemental Reports (Table G2 and Table K4) Actually Show for NPWT vs. Surgical Dressings and Orthoses

Because CMS's Medicare Provider Compliance Tips — Negative Pressure Wound Therapy web page (last modified February 11, 2026, verified in the preserved primary-source extract) still quotes only the 2024 CERT rate of 17 percent and ~$12.1 million (s12), while 91 FR 47972 (published July 30, 2026, s1) incorporates the November 2025 CERT Supplemental Report (s11), tender writers and compliance officers must read both official CERT reporting years side by side with their sample sizes (Claims Reviewed) and 95% confidence intervals (CIs) 1 10 11 12.

Table 6 compiles the verified Table G2 (Improper Payment Rates by Service Type — DMEPOS) and Table K4 (Upcoding Improper Payments) rows for Negative Pressure Wound Therapy from the 2024 CERT Supplemental Report (claim window July 1, 2022 – June 30, 2023, the preserved primary-source extract) and the November 2025 CERT Supplemental Report (claim window July 1, 2023 – June 30, 2024, the preserved primary-source extract) 10 11.

No formal between-year test is supplied here; interval overlap alone is not a valid difference test 10 11.
November 2025 CERT Supplemental Report — Table K4 (NPWT Upcoding)July 1, 2023 – June 30, 202445 (policy group sample)$501,3640.9%(-0.8%) – 2.5%0.0%The 95% CI for upcoding (-0.8% to +2.5%) includes zero, reflecting uncertainty in the estimated improper-payment rate; it does not imply negative actual payments 11.

Table 6: Verified Medicare Fee-for-Service CERT Supplemental Report statistics for Negative Pressure Wound Therapy (2024 Table G2 vs. November 2025 Table G2 and Table K4) 10 11 12.

Three limits govern interpretation of these estimates. First, the 17.0% and 11.1% figures are estimated improper-payment dollar rates, not the percentages of sampled claims that were incorrect. The sample counts and confidence intervals should accompany comparisons. Overlapping intervals are not themselves a valid test of the difference between estimates. No formal between-year change test is supplied here, so the report describes the point estimates without claiming an established improvement 10 11.

Second, the displayed 0.0% share is rounded and coexists with positive projected improper-payment dollars. It does not mean NPWT generated zero improper payments. No NPWT-specific root-cause table was located in the inspected supplemental tables; this observation does not prove CMS's reason for omitting one. The current compliance-tip example identifies insufficient documentation, including missing evidence about nutrition, diabetes management and offloading; it should not be rewritten as a case about missing monthly wound measurements 12.

Third, the Master List notice identifies the aberrant-billing pathway for A6022 and A6197. It does not establish stable E2402 utilization, prove that a particular supplier committed fraud, or show that an individual claim is noncovered. Master List retention and later selection depend on the governing conditions. Better supplier records support accurate claims and audits, but cannot guarantee that CMS will not select a code for future F2F/WOPD or prior authorization 1 13.

7. Summary Conclusion: Precision Across Adjacent Codes, Lists, and Benefit Channels

The useful commercial distinction is between a product code, a coverage condition, a fee allowance, and the payment route. E2403 and A6614 do not become wound-care codes because they sit near NPWT entries. Master List inclusion does not itself impose all downstream conditions. A bidding limit is not an awarded price. A home-health device rate cannot be moved into an office or DMEPOS column without changing its meaning. These are potential worksheet errors, not a claim that their market prevalence has been measured.

For a consumables supplier such as VEMERIX, the practical deliverable is a product-specific evidence pack: accurate identity and dimensions, intended-use documentation, applicable coding verification, and current policy links, with commercial price kept separate from payer allowances. The purchasing or billing organization should confirm the actual benefit and claim requirements. A structured inquiry can identify missing documentation before a tender is finalized; it cannot guarantee coverage, payment, or a particular audit result.

A robust internal worksheet can preserve the original downloaded amount, source date, code and modifiers alongside a separate interpretation column. Record who selected the care setting and geography, whether a rental amount is for the initial or later stage, and which coverage requirements remain unresolved. If a source changes, the original and revised assumptions should be distinguishable. This is a recommended control for preventing silent spreadsheet drift, not an additional CMS form or legal filing requirement.

For an example, a buyer comparing a reusable pump with a disposable system should first establish which actual products and settings are being compared. The durable device can involve capped rental plus separately coded supplies, while the disposable home-health device has a setting-specific payment route and treatment of visit time. A one-month comparison that ignores rental stage, supply frequency, eligibility, or noncovered items can create a misleading cost advantage. The available files can support the relevant allowance inputs, but they cannot supply the buyer's clinical utilization assumptions or the supplier's acquisition price.

When evidence is incomplete, mark the unresolved field rather than filling it with an adjacent code or a secondary summary. The appropriate next step may be checking the current policy article, confirming PDAC classification, obtaining the beneficiary's setting and geographic information, or requesting a current commercial quotation. Keeping those questions separate makes the comparison more useful to both procurement and reimbursement teams and avoids turning a legitimate published number into an unsupported payment promise.
